COMPLETING A WEB SURVEY

1.      Sample web surveys are posted at: http://webhome.crk.umn.edu/~nelson/P&A_Review.htm

 

2.      A sample memo informing those to be surveyed about the web evaluation form and assigning them their unique PIN # is posted at: http://webhome.crk.umn.edu/~nelson/surveys/P&AEmailMemo.htm . This document is to be sent as an e-mail using the mail merge feature.

 

3.      A sample Excel file listing the names and assigned PIN #s of those asked to complete the survey is posted at: http://webhome.crk.umn.edu/~nelson/surveys/P&APins.htm

 

4.      Tutorials (prepared by ITC staff) describing how to use mail merge to send e-mail notifications are posted at: http://webhome.crk.umn.edu/~nelson/P&A_Review.htm

 

Should you choose to use the web survey you will be responsible for:

1.      Creating your survey questions in a Word document and e-mailing it to me. Please do not number the items or use any special formatting.

 

2.      Creating your version of the memo to be e-mailed to those you will ask to complete the survey.

 

3.      Creating your Excel document listing those to be surveyed and assigning their unique PIN #. Survey requests are not to be sent out to the Student and Faculty/Staff distribution lists.

 

4.      Completing the mail merge sending out the e-mail. I will provide you with the unique URL to be used for your survey.

 

5.      Completing any follow-up with non-respondents.

 

I will be responsible for:

1.       Creating the web survey from the Word document you provide. You'll have the opportunity to proof this.

 

2.       Sending you the final URL for the web survey to be used by respondents.

 

3.      Providing you with a specific number of randomly assigned PIN numbers. You need to tell me how many you will need.

 

4.      Tabulating the final results and providing you with the responses as a Word document. (Excel file of responses is optional).
