Instructions: This questionnaire is being sent to UMC audiences for the purpose of evaluating Dr. Ariz Rahman in accordance with University of Minnesota policy for contract faculty performance review. Assessment information will be solicited from a wide spectrum of people who interact with Dr. Rahman. Summary of job description
Dr. Aziz Rahman develops and teaches courses, advises students regarding both specific coursework and broader academic or career goals, engages in research and publication, and/or artistic or scientific creation, provides service through participation in governance of the department and/or the University, contributes to the development of the profession through participation in national and/or international associations, publications, and/or other related activities, continues personal growth within the profession, keeps current with the field, and broadens knowledge and expertise, and contributes to outreach activities that support the unit’s mission.

University policy and the Minnesota Data Practices Act requires all survey responses to be individually identifiable (with the exception of students and staff that report directly to Dr. Ariz Rahman), and will become a part of the official committee file. The number that appears on the e-mail requesting you to complete this evaluation and the "key" associating the PINs with individuals will only be accessible to the Committee Chairperson, the immediate supervisor Dr. Richard Nelson (Agriculture and Natural Resources Center Director), and, if requested, by the individual being reviewed. Responses from students and those reporting to Dr. Rahman will have the PIN number deleted after verification that it is a valid response.
