INSTRUCTIONS: This questionnaire is being sent for the purpose of evaluating Dean McCleary’s work performance. A periodic comprehensive review for full-time Professional/Administrative (P/A) staff is one factor that is used by administrators in considering reappointment decisions. Assessment information will be solicited from a wide spectrum of people who interact with Mr. McCleary including faculty, staff, and students from the UMC campus and selected individuals outside the campus.


The Vice Chancellor for Finance areas of responsibility include: financial policy and budget process and financial operations. The areas the VC for Finance supervises include the Business Services Office (Purchasing, Inventory Control, and Payroll), Bookstore, Office of Human Resources, Postal Services, and Financial Aid.
The Review Committee would very much appreciate your completing this evaluation by April 5, 2002.

POLICY REQUIREMENTS
University policy and the Minnesota Data Practices Act requires all survey responses to be individually identifiable (with the exception of students and staff that report directly to Dean McCleary), and will become a part of the official committee file. The number that appears on the e-mail requesting you to complete this evaluation and the "key" associating the PINs with individuals will only be accessible to the Committee Chairperson, the immediate supervisor Dr. Don Sargeant, and, if requested, by the individual being reviewed. Responses from students and those reporting to Dean McCleary will have the PIN number deleted after verification that it is a valid response.
