INSTRUCTIONS: This questionnaire is being sent for the purpose of evaluating Russ Kreager's work performance. A periodic comprehensive review for full-time Professional/Administrative (P/A) staff is one factor that is used by administrators in considering reappointment decisions. Assessment information will be solicited from a wide spectrum of people who interact with the individual being evaluated including faculty, staff, and students from the UMC campus and individuals outside of UMC.Please complete the following questionnaire by May 11, 1999.
