INSTRUCTIONS: This questionnaire is being sent for the purpose of evaluating Russ Kreager's work performance. A periodic comprehensive review for full-time Professional/Administrative (P/A) staff is one factor that is used by administrators in considering reappointment decisions. Assessment information will be solicited from a wide spectrum of people who interact with the individual being evaluated including faculty, staff, and students from the UMC campus and individuals outside of UMC.

1. University policy and the Minnesota Data Practices Act requires all survey responses to be individually identifiable (with the exception of students and staff that report directly to Russ Kreager), and will become a part of the official committee file. The number that appears on the e-mail requesting you to complete this evaluation and the "key" associating the PINs with individuals will only be accessible to the Committee Chairperson, the immediate supervisor Dr. Robert Nelson, and, if requested, by the individual being reviewed. Responses from students and those reporting to Russ. Kreager will have the PIN number deleted after verification that it is a valid response.
